This questionnaire is designed to help us get to know you and your fiance better. We hope in this process to set you up with a mentor couple who will best serve your needs as you begin your marriage journey. This questionnaire is confidential and will be seen only by the Marriage Pastor & Marriage Team. Please answer every question and be as thorough and honest as possible. We need to receive a separate form from both the Groom and the Bride. Once we receive the completed forms, we will contact you to set a time to meet with you and arrange for you to begin the process.
